In the settings for each separate extension you're sometimes and to change from romaji to english title language, but this breaks the tracking on some animes. Since you manually select what you want to track in aniyomi, it's only a problem in cloudstream.
There's always gonna be shows that fall through the cracks when automatically matching titles but its currently a lot of shows and the easiest method to fix this would probably be to have the extension show titles in english if the title language is set to english, but still track them by using the original title you get from the romaji setting.
Id prefer a way to manually pick titles like aniyomi or like dantotsu has if it happens to pick the wrong title, but i think this could be a compromise that works for the most part and is still fairly simple for you to implement.
If you want to confirm this or try it for testing purposes, you can replicate it by using the kaido extension, set title language to english and try playing any of these animes.
