Add Command Centers, Locations #158

Add form to add command centers, shelters, headquarters, Info Centers, other Location. By default, these locations should be be public (that is, on a public layer that others can turn on and off.

The form should ask for:

Name/Title of Location
Type of location [command centers, shelters, headquarters, Info Centers, other Location]
Primary contact person
Address
Phone Number
Lat/Long
Start Date for location
End Date for location
Share this location with others?

[Save] [Deactivate Location]
