Updated README with instructions on updating redirect_uri in the db #52

If the hostname is changed, the oauth_clients table needs to be updated. We might want to have a script for that at some point, but for now I added instructions and a sample query to the README.

I have merged this PR, adding a note about how Mattermost build the redirect_uri parameter.
