SUS doesn't display properly on Chrome, because of interaction between shinyDashboard and reactiveVariables #20

We need to drop shinyDashboard and remake the top-level UI with a navbar.

This also means relocating the title bar a sidebar, which allows us to consolidate all the module-specific controls in one place.

This has been fixed with the development version of mapdeck.
