As a known Patient I can request a prescription online so that saved time for both clinik and mine

In order to receive benefit as role, I can goal/desire

Acceptance Criteria:

1 My informations entries on a form.

2 With one click submit the information

3 feedback if successful.

Tasks

task 1 : inputs boxes for f_name ,l_name, email, object, status, and submit button

task 2 : submit with one click

task 3 : info send as email to an official email.

task 4 : feedback if successful/unsuccessful
