Adding of Questionitem in Concept

Just now a QuestionItem dissappears when added to a concept

I did not modify the related code today. It works when I tested adding a question item to Concept (c1) from Topics(Health Inequalities 2 based).

I tried to add one question to the same concept, but when I reload it disappears

When I add "nnnn ttt", the backend throws:
