Show ban reason #48

When you ban a user (profile page) you are prompted with a display to enter the reason for the ban so other staff members can read it if they visit the profile and deal with a ban dispute where the reason is sometimes ambiguous.

Ban reasons can be put in the permissions page, or at least note who banned them.
