Chord analysis when the melody is in the bass #11

This is one random example of many that would be available: image\ K457-1, mm. 133f.

Annotators generally analyse the inversions as caused by the melody tones, simply because they are the lowest sounding notes. However, this often does not coincide with what can be heard; there may be instances where the actual bass voice is not the lowest one. Should we write about this in the guidelines? And what?

More (difficult) examples welcome.

Depending on texture, I'd rather think of the voice that carries the main harmonic function as the one where the annotation should spring from. Again, this relates to stylistic features. Same about organ points: what if the stable note is in the highest voice? One hears an organ point still.
