Description

The issues are as follows, ranked by size

Extra Small

[ ] Specify outcome chosen in the message. 'You have submitted a pass/fail for [teacher name].'
[ ] @JSJohal a fail is a pretty serious thing to submit, we're adding a confirmation page before the submission but could the UI of 'success! you've submitted...' be different if it's failed?

Medium

[ ] Create a confirmation page before an outcome is recorded. The page should include:
[ ] Title - Please confirm your details
[ ] Summary of data being submitted in a table
[ ] Continue button
[ ] Back button

Did @peteryates suggest having this as 2 separate flows?

There was a big chat about it on the bug party. The idea that two important outcomes were very close together worried people.

We decided to have two screens for this and make it really clear that one's for pass and one's for fail.
