Specification and Guideline should mention OpenEHR #16

From Stockholm County: "OpenEHR is a standard used for patient-centered health data and records, but not mentioned at all in the Specification or in the Guideline. Why is that?"

OpenEHR is a data model used for creating EHR systems, FHIR is a data exchange/interoperability framework. However, mention shoukld be made for clarity.

Added description of openEHR and ISO 13606 to Guideline, reduced emphasis on HL7 FHIR.
