Review comment (Ch. 5.4.2): Revise the usage of ":" and "+", should some of the ":" characters be "+" characters.

The identifier "urn:uuid:123e4567-e89b-12d3-a456-426655440000" is converted to "urn+uuid+123e4567-e89b-12d3-a456-426655440000" according to the pairtree specification. It depends on if the "identifier (uuid)" or the folder name is used.
