Add additional fields to the UserForm2 and the tsd_table_1 per the TSMS requirements.

This task involves the following:

Expand UserForm2 and add the required GUI widgets per the TSMS requirements (see the Test_Scheduling_Manager repo).
Modify the Data tab to accommodate the additional fields (columns).
Remove items in the Data tab that are no longer needed.
the tsd_table_1 updates will be tracked in the Test_Scheduling_Manager repo and are to be handled via _Convert_Job_Sort_csvdata to sql.py and the DB Browser for SQLite.
