Block creation: Pending transaction with nonce too low,

what is the origin of this error:

Are the transactions with the "Pending transaction with nonce too low" only because they got just imported in the block before ?
are they in the "Transaction Queue" with a Nonce Gap ?
is it a transaction without nonce gap ?

todo: improve logging with more infos about the transaction.
