What version of the DMPRoadmap code are you running? (e.g. v2.2.0) v3.0.2 test.dmponline.be june 2021

Expected behaviour:

Include DMP export eg in pdf format when sending feedback request notification by mail to RDM support staff OR alternatively log version of DMP on which feedback has been provided (see also issue #31 )
Allow feedback requester to include optional comment when requesting feedback. eg to point out sections of the DMP that require additional attention
Allow feedback provider to pass on document (word or pdf) when performing or completing feedback
include email address of feedback requester in feedback request notification to RDM support staff

Actual behaviour:

No track of version of DMP on which feedback has been provided; or of feedback (comments can be deleted)
Feedback request is not customizable
Not possible to include document in feedback (eg review rubric)
user name, but not mail address, is includede in feedback request notification to RDM support staff

Steps to reproduce: Request feedback for a plan, then complete feedback for a plan by clicking 'completed' in admin > plans view
