System: CentOS release 6.5 (Final) PHP Version 5.5.16 Magento 1.8.1. One Step Checkout 4.1.3 error002

Other extensions being used: Idev_OneStepCheckout N98_CustomerGroupCheckout DPD_Shipping PostcodeNl_Api Sisow MultiSafepay_Msp ParadoxLabs_StorePickup

Tested with each extension installed one after the other, couldn't reproduce. Suspected server permissions problem.
