Noticed that the docs are outdated, specifically the README missing certain sections.

Other cleanup:

Add issue template to the repo
Make sure it everything is community standards compliant: https://github.com/DSACMS/repo-scaffolder/community

still debugging. Got Repolinter output to work, having issues with GH-actions creating an issue on the repo. Permissions/Tokens? We're still troubleshooting.
