Tier 0: Template Review based on maturity model updates

Problem

Due to updates made to the maturity model from outbounding DedupliFHIR, the Tier 0 templates need to be reviewed to ensure they align with newest changes.

Solution

Added Recommended sections to README.md
Created CONTRIBUTING.md and added Recommended sections
Cleaned up templates

Yes, tier0 is likely a solo-repo, and would not include a team.
