Upload: Add button and panel to upload survey #162

A small MR to add the upload button and panel. This can't work for now (unless combine with #97) but i check (using cherry-picking) and the upload works properly. Let me know if i should change the css tho

I would say that the system spec is the priority, but if you have time you can look at #163 #157 (if you start something don't forget to assign it to yourself :))
