Preserve filename #222

When the CD is not in the external database f.ex Amazon (no metadata 
available), there should be an option to preserve original filenames when 
transcoding. Now these are replaced with Track 01, Track 02 etc. and then I 
have to rename manually afterwards.

The problem is, the original filename does not always exist; e.g. ripping from 
a CD or transcoding from a single wav + cue.

If you want to preserve filenames, set format of filename option to default, 
open files (without .cue, not "open folder as a disc"), and convert.

If the original filename doesn't exist, the application can (and should) 
display a warning or error during the conversion step. The default naming isn't 
always appropriate for converting from an existing file, either.
