Manual guide dialog enhancements #231

- add a dither button
- add ability to specify pulse duration
- add some visual indication that a pulse is active
- (bug) there should only be one instance of the Manual Guide dialog (currently 
a new window opens each time you select Manual Guide)
- get rid of the message box that says "cannot manual guide when guiding or 
calibrating", but still allows you to do so
