enhancement: better use of subframes for slower cameras #241

when subframes are selected phd2 uses full frames during looping and 
calibration, then switches to subframes for guiding.

for cameras with slower download rates, it may be unacceptably long to use 
subframes during calibration

note: phd1 starts with full frames, then switches to subframes after a star is 
selected, so it uses subframes for calibration
