TOC Considerations #16

Perhaps a slight re-design of the concept of the TOC. Something inspired by https://onsen.io/guide/overview.html would be nice.

Since this seems to be the best place to put this:

I've also considered adding a printToc variable in the front-matter of any given page that will determine if the toc is shown during a printout.

Along the same lines, we might also allow the maxTocDepth to be set in the same place.
