Added notebook creating files with the druguse definitions #54

This is the notebook that belongs to read_data.py, which pulls a bunch of data files from CMS and identifies drugs with generic classes.

One of these days, I will have two hours to upload these files to data.world and update the notebook, but tonight is not that night.
