It appears when you deselect "label" or "description" it still shows when you output the form using shortcode.
Not sure if that was added in there layout wise and functionality still needs to be added or if it's a bug, but wanted to point it out.
Figured this out when I was attempting to set the description as the placeholder, and then remove the description from showing on the form output.
