Some incorrect timers: Gouge, Calamity, and then Defile Ground and Vengeful Strikes.

Did you try having DeadlyBossMods as the only enabled addon and everything else (especially something like ElvUI) disabled? No.
Which version of DeadlyBossMods are you using? DBM-9.1.20-14-gd6ccaa0 MoP r144-1
Was it working in a previous version? If yes, which was the last good one? I don't know.
