Fixes #57 Introduce propulsionref element

Ok. I've added a propulsionRef. Are you able to make it so that we actually link to #propulsion in the target document?

Oh, and to include propulsion in the in-page table of contents. My favourite outcome would be for the table of contents to link to the target content, not the propulsionRef in the current document.

Hmm, I don't think it's right to remove the out folder, since that's what gives us a browse-able version.

The output seems to be the output before switching to propulsionref. I see propulsionRef in the .dita file, but not in the outputl

Sorry. I didn't need the element changed. The case change was a typo. It's not actually an issue. I was looking at unit_db in dita, and unit_a in html. Oops :-(
