Remove left-hand table of contents #62

The structure/content of the existing document includes navigation (world image-map, clickable links, table of countries, table of classes).

So, the left-hand TofC becomes a waste of screen space.

We should remove it. Hopefully we can do that via some property in the scenario.

If we don't have the left-hand table of contents, and all navigation is via in-page links, then I don't think we need to maintain the ditamap of content within the regions folder. The home page just need to include a reference for regions.dita - and navigation happens from there.

I've just checked, and the WebHelp process is indexing files that aren't in the ditamap (such as french power units). So, it looks-like we don't have a strong reason to maintain the region/country ditamaps.
