Convert `summary` and `signatures` sections to extend `section` #79

The in-page table-of-contents is a really valuable feature: image

But, it only captures elements that are based upon <section> - so it doesn't include summary or signatures

Could we please change the DTD for summary and signatures so that they are based upon section?

We'd still like the elements within them to be constrained in the same way that they currently are.

I'm expecting the outcome of this issue to be a PR in this Fi3ldMan repo.

Note: it looks like this is fixed, but they aren't showing in the local menu. @IanMayo to check if it's because the title is missing.

Done - I just needed to include the section titles
