8 years ago hopefully that fixed it. It wasn't the SD read that caused the problem, as it can loop through the images quickly, without any problems.
I've tried to make the code more robust:
- radio ACKs are sent before processing the message content (as all the failures coincided with an ACK not returned)
- we make checks that a file exists on the SD before reading from it (as the failure occurs around the time an SD image would be read
- on a failure to open an SD card file, we try closing any file before reopening
I haven't had any of the (error3) failures during testing today.

I suspect that it's falling into a hole to do with loading files from the sd card, but this needs some invesigation to sort out.
