Rebrand the module from thycotic to delinea

• Updated the readme badges to reflect the new GitHub location
• Updated the license to MIT
• Added templates for issues and PRs
• Added code of conduct
• Regenerated the docs, merging in a rebranded example in the index

@delineaKrehl in some places the version is 1.0, others 1.0.2, and some 2.0. Do we want to standardize on 2.0 throughout the project?
