[25330] Ability to select/create/remove a request type when creating/updating a Form

Upon creating/updating a specific form, there is no option to select a request type to associate with it from the Forms page.

You will need to go to Portal > Request Types, then add/update a request type to assign a form to it.

It will be handy if we can also create/select/remove an request type from the forms' page, much like we can select/edit/remove a form from a request type's page
