"Siren - Ward of Despair" missing reward

[Hell-No-Kitty]

I didn't receive the "Miyako outfit" from the "Siren - Ward of Despair" space after completing the quest and getting the notification that the items had been added to my inventory. Siren has 3 separate quests, and I received the rewards from quests 1. and 2. into my inventory as expected:

1. Doctor's outfit quest
2. Nurse's outfit quest

I'm sorry, I should have been clearer. These 3 items, below, combined make up the Miyako outfit. (top, skirt and boots).
