using two 5850 with v2 return 100% hw error. at @250~300Mhash/s each

Sounds like a driver bug. Try reproducing this in cgminer.

already tried a clean install with driver sweeper.

What has to do cgminer? there are another prerequisite apart from clean driver?

cgminer is another OpenCL miner

None. If you had done something wrong, it wouldn't have started up at all.
