Our current practice is to use ref for URIs, and to encode the URI both as the value of @target and as the content of the ref element. This system is cumbersome (and our current display stylesheets already display the value of @target, even if the ref element is empty). In addition, there may be accessibility concerns. We should revisit our practices and reconcile the encoding, the stylesheets, and the schema.
