Pointing to images of manuscripts #6

I think we need to add <additional> with <surrogate> to the part of the schema that deals with manuscript description so that editors can point (using <ptr>) to images of manuscripts.

Better to use idno with @type="URI" instead.

Updated guidelines so that altIdentifier can include idno with @type="URI" to point to digital representations of manuscripts.
