Italic styling of <note> #23

All text in <note> is displayed in italics, but there should be a way to differentiate between the different kinds of text. For example, a reading should be in normal type, but editorial comments should be in italics. Currently there is no way to make that distinction, even when I encode a reading with hi rend="normal">.

Add a CSS rule for @rend="normal" canceling italics, etc. See 1.18.
