Design Request Form #16

The request form has only simple HTML code, can I add some CSS responsive codes

templateform.css was not linked correctly in the form.html So, I changed that and did some other changes in adjusting the text. Here is pic of request form page: request-form If it was fine, then I will merge all the changes.
