Maintenance updates for console.dart

Made a number of minor changes to improve this package's score on pub.dartlang.org, in three categories:

Added some documentation (an index of the examples, a CHANGELOG, a longer description);
Fixed a few minor bugs in the examples, mostly relating to the switch to a sound type model in Dart 2 (also updated the questions in the Dart quiz for accuracy :)
Ran dartfmt --fix on all Dart code to remove unnecessary new and const keywords and switch to recommended syntax for named parameters.

After incorporating these changes, console should score 100/100 for health and maintenance.
