correct url for canada #67

I wasn't able to log in to Canadian d+, and figured out Canada uses the US url. Not sure if this is the best place for it to be set or not, but it's working for me.

Hi, thanks for PR. I am exploring a better way to handle different countries. So i don't accept this yet.

Support for d+ Canada is now added in version 1.6.5
