Switch to new modals and attachments, brand new README and setup instructions #36

This PR switches away from the legacy "prompt" system to modals and attachment arguments for many things, such as for adding new tags.

Also supported are buttons for tags.

The README has also been almost completely rewritten to make setup more intuitive.
