these are very general terms, intentionally so I think.
chronic cardiac disease - in DO: heart disease & child terms
chronic pulmonary disease - in DO: lung disease

Some of the most common are
chronic obstructive pulmonary disease (COPD),
asthma,
occupational lung diseases and
pulmonary hypertension.
chronic hematologic disease. - in DO: hematopoietic system disease chronic neurological disorder. in DO: nervous system disease
mild and moderate to severe liver disease - in DO liver disease, the stages are not different diseases

The WHO COVID patient data sheets list a number of comorbidities that are not currently readily mapped to DO:
chronic cardiac disease - complicated as the WHO term excludes hypertension chronic pulmonary disease - complicated as the WHO term excludes asthma chronic hematologic disease
chronic neurological disorder
The form also distinguishes between mild and moderate to severe liver disease but I don't think this is an appropriate distinction for DO
Do we want to add these? The last two seem relatively straightforward but the exclusions in the first two would be difficult to fit in the DO structure. We might have to go with a broader term that ignores the exclusion.
