Allow KOtal to be globally disabled, but switched on for individual views. #8

I've added a new key, 'enabled', to the config file, cached that value in the View class, and added documentation for it.

The reason being that I didn't find the controller-level exclusions fine-grained enough.

It's still on by default, but you can now switch off KOtal globally, and just "switch on" certain views as you convert them.
