Update measures to accomodate example MPOG data

To successfully process the data flow from MPOG, measures.json needs to have entries for all of the measures being used by the example data flow.

For each of the measures listed in the data flow (second sheet)

• If an existing measure, check for completion
• If novel measure:
  • [x] Add '@id'
  • [x] Add "@type"
  • [x] Add "Dc:title" from MPOG database
  • [x] Add "identifier" from spreadsheet, verbatim

Of note, some measures have names that include terms like Peds, Card, etc. Working with MPOG to gain clarity on the differences between the public spec database and internal measure names.
