Laura/Allison will be mailing out info to participants on October 20th, so we need to have this done by then.

Looks great! We might also consider a text box asking them if there is anything in particular they want us to cover in the course. This has pros and cons, of course. The main pro is that it would help us ensure we deliver what they want. The main con is that they might bring up something that we don't want to cover, and by knowing about it ahead of time, it puts in the position of specifically choosing not to cover it, rather than being able to plead ignorance. (Which isn't a very strong plea...)

Okay, I'll add a box in the last page. I agree that a plea of ignorance is not very good and presumably folks will ask anyway. I'll close this issue and send the link to Allison and Laura.

I was too slow to respond to the questionnaire before you closed the issue.

Why are you gathering these data? So you and Jason can "take appropriate action". How much time will you need to "take appropriate action"? Several days I would assume. In any event, you do not specify on the form or its instructions WHEN you would like the completed form.
I would expect should have participants complete the form before they begin their journeys to Duke; in the case of most participants, I expect that to be over the weekend.

Hence, you ought to ask for the forms to be completed by Friday the 23rd, so that a) folks don't forget to do it and b) you get information in time to act on it.

Good point. Sorry Eric, you got missed out in the discussion Jason and I had with Allison and Laura about this. In their mailout they'll ask participants to fill in this survey by Thursday, so we have the information over the weekend.
