Manuscript improvements

handles two additions and complementary documentation coming from manuscript review:

• auto create dead letter queue
• auto create (and cleanup) dashboard

note that for dead letter queue I've changed config input to name instead of ARN as if it is not made yet it won't have an ARN yet.

Nonzero chance this will have to be rebased now that #13 is pulled, but let's see how we go!

Can you also add a line or two to wherever seems reasonable (guessing somewhere in the docs and somewhere in the readme) to point folks to DHW for an example?
