Upload Image button not working

What steps will reproduce the problem?
1.  Edit a page
2.  Click the "add image" button and select the upload tab
3.  Select a file on your computer and click "Send to server"
4.  An error is displayed "Server error"

What is the expected output? What do you see instead?

Should see success message and file should be uploaded correctly.  Work-around 
is to upload the file from the resources screen which is still working.  This 
is with version 9.12
