Preview merge: misc merge train #528

Several commits in this train refer to large improvements to the syscall/filesystem layer:

Lifted maximum of 4 open files in dragonfs
Improved performance of the syscall layer with O(1) lookups of open files
Reduce preallocated file descriptors by using a growable data structure (and also bump limit to 4096 open files)
Improved error reporting in SDFS by correctly setting errno in all calls

There's also some commits in the mixer and some vendoring in preparation for merging VADPCM support to trunk.
