Could you please show me a log file after this happened?

Log file here:
http://pastebin.com/Pd8c78Xq
Situation:
Started FIFA 98 and quit.
Mega Man Soccer automatically launched after I returned to RCB screen. I quit
Mega Man Soccer. Mega Man Soccer restarted after returning to RCB screen.
Repeat forever.
I think it might be due to the way I'm mapping my exit game key with ZSNES?
I'm using an XBOX360 controller with xboxdrv. I'll try to re-map it tomorrow.

Re-mapping has not fixed the issue. It seems rather random when it will exit
normally, and when it won't. I'm kind of clueless at this point.

Sorry, I can't see anything obvious in your log file. It looks like a launching
process on my system, just without launching the game twice. I really guess it
has something to do with the button mappings but I can't say for sure.
I have seen these two lines in your log:
00:30:08 T:3054839680 M:2911145984 ERROR: Keymapping error: no such action ''
defined
00:30:41 T:2826902384 M:2947641344 ERROR: Previous line repeats 6 times.
Maybe this causes an error?
You could also come to the thread in the XBMC forum and ask your question
there. Maybe somebody else had a similar issue and found a solution.

Same exact issue. Any advice?

Sorry, no. Does it also happen if you don't map any exit actions for your
controller?
Maybe XBMC is still receiving input during you play the game. You could try to
go with RCBs solo mode or try LIRC.stop / start commands (or whatever handles
your input).
Maybe this helps: http://forum.xbmc.org/showthread.php?p=1001488#post1001488
