Improve the functionality of the multivariate missing data visualisation #16

The multivariate missing data visualisation is useful, but it might be an idea to also look at the top 5 or 10 combinations of missing data in the event that there are a lot of combinations.

It may also be worth increasing the physical size of the first plot we produce also.

This needs a revisiting - there are some logical errors in the proportion calculation and I also want to improve the efficiency of the calculation - perhaps giving an option to only display the larger proportions of missing combinations?
