@joliet the CIT contact field doesn't make sense if we post this form to the gateway page. What do you think?

Sure. I designed the form to be used internally, as a quick way for consultants to keep track of improvements they heard from faculty. The CIT contact field gives us a name to follow up with if we had questions or wanted to loop in a consultant for more information. If you'd like to share this form publicly, I suggest we keep the CIT contact part to track consultants for requestors, but add a new field for email address for the requestor, for follow up. We can look folks up, but as you know, that can be tricky sometimes based on the info in the directory and the info provided.

As long as it's clearly labeled so people who aren't CITers know not to care about that field, sounds good to me. Let me know when it's ready and I'll date the code here.
